Job Purpose:

To provide and coordinate the provision of skilled nursing care under physician supervision, through competent application of nursing process.

Key Responsibilities:

1. Provides competent nursing care & interventions to home health patients according to state, industry, and professional standards.
2. Supervises care provided by Home Health Aides and Licensed Practical Nurses (LPN), including supervisory visits of HHAs as required.
3. Collaborates with the interdisciplinary team and attending physicians to develop, implement, and manage the patient care plan, making referrals as needed.
4. Demonstrates intermediate knowledge of home care and competency in discipline-specific patient care skills.
5. Conducts field visits to evaluate, initiate start of care, and monitor clinical outcomes and care performance.
6. Utilizes approved teaching materials for patient and family education.
7. Assesses, monitors, documents, and reports patient health progress using required documentation and technology within established timelines.
8. Completes accurate documentation at the point of care, including assessments, care plans, physician orders, and care coordination.
9. Honors patients' rights and works with the team to adhere to care plans, ensuring equitable treatment and respecting privacy and civil rights.
10. Complies with corporate policies and safety regulations.
11. Meets productivity standards and utilizes resources effectively.
12. Participates in surveys, in-services, case conferences, and staff meetings as required.
13. Maintains current licensure, certifications, and clinical records in compliance with regulations.
14. Demonstrates self-direction, prioritization, and time management skills to meet outcomes and deadlines.

Minimum Education:

• Nursing degree from an accredited college/university or graduate of an approved RN program.

Minimum Experience:

• At least 1 year of RN experience.
• Valid driver's license and reliable transportation.
• Liability insurance for home visits.

Licensure/Certification:

• Current, unrestricted RN license in the state of practice.
• CPR certification.
