Job summary

CHI St. Vincent Health at Home is seeking a registered nurse to deliver high-quality home health care in the Clarksville service area. You will lead clinical care, develop personalized plans, and collaborate with physicians, therapists, and support staff to ensure holistic patient outcomes.

You will educate patients and families, mentor bedside staff, and utilize advanced documentation tools to maintain accurate records while supporting a patient-centered, compassionate care environment.

Qualifications

• Graduate of an accredited nursing program with an active RN license in the state served.
• Minimum of 1 year of nursing experience (Home Health experience is a significant plus!).
• Exceptional critical thinking, proven clinical expertise, strong case management abilities, and an unwavering commitment to patient-centered care.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.
