Job summary

CommonSpirit Health at Home is seeking a Home Health RN to deliver patient-centered care in the patient's home. You will have a 1:1 patient-clinician ratio, ample time with each patient, and a chance to witness the full arc of healing.

You will lead clinical care, craft individualized care plans, and collaborate with a multidisciplinary team. You’ll educate families, mentor LPNs and aides, and leverage technology to ensure accurate documentation and optimal outcomes.

Qualifications

• Graduate of an accredited nursing program with an active RN license in the state served.
• Minimum of 1 year of nursing experience (Home Health experience is a significant plus!).
• Exceptional critical thinking, proven clinical expertise, strong case management abilities, and an unwavering commitment to patient-centered care.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.

$35.53 - $52.85/hour
