Access Home Health is seeking Registered Nurses (RN) for full-time and PRN positions to provide one-on-one home health care. Qualifications

• Active Florida RN license and CPR certification required.
• Proven strong assessment skills and clinical judgment.
• Home health/OASIS experience preferred but not required.

Responsibilities

• Conduct comprehensive assessments of patients’ health needs and develop individualized care plans.
• Administer medications and treatments as prescribed, monitoring for effectiveness and side effects.
• Provide education and support to patients and families on health management and prevention.
• Perform wound care, catheter care, and other nursing procedures as needed.
• Document progress in Epic and ensure regulatory compliance.
• Collaborate with physicians and therapists to coordinate care and maintain safety.
• Participate in ongoing professional development and training.

Compensation Structure

• Salary range: $80,000 - $110,000/year Salary Range: 85,000 - 120,000
• Start of Care (SOC): 2.0 Units
• Evaluation / Re-Evaluation /Recertification / Resumption of Care: 1.5 Units
• Routine Visit: 1.0 Unit
• OASIS Discharge: $65

• Ensure infection control protocols are followed to maintain a safe environment for patients.
• Participate in ongoing professional development and training opportunities.
