Job summary

CommonSpirit Health at Home is seeking a Home Health RN for Teller County. This PRN role focuses on delivering high-quality, in-home nursing care with a 1:1 patient focus and autonomy in clinical decision-making.

You will develop and adjust care plans, collaborate with a multidisciplinary team, educate patients and families, mentor LPNs/LVNs and HHAs, and leverage technology to improve outcomes. Requirements: RN license in state served, minimum 1 year home health experience, CPR/BLS, valid

Qualifications

• Graduate of an accredited nursing program with an active RN license in the state served.
• Minimum of 1 year of nursing experience. Home Health experience required.
• Exceptional critical thinking, proven clinical expertise, strong case management abilities, and an unwavering commitment to patient-centered care.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.

Job Requirements
