Job Purpose

To provide and coordinate skilled nursing care under physician supervision, applying nursing process competently.

Key Responsibilities

1. Provides competent nursing care & interventions to home health patients according to state, industry, and professional standards.
2. Supervises care provided by Home Health Aides and LPNs, including supervisory visits as required.
3. Maintains continuity of care by collaborating with the interdisciplinary team and attending physicians to develop and manage patient care plans, making referrals as needed.
4. Demonstrates intermediate knowledge of home care and competency in patient care skills.
5. Conducts field visits to evaluate & initiate care, monitor clinical outcomes, and participate in care improvement initiatives.
6. Uses accepted teaching materials for patient/family education.
7. Assesses, monitors, documents, and reports patient progress using required documentation and technology within established timelines.
8. Completes documentation accurately and timely, including assessments, care plans, and physician orders.
9. Respects patients’ rights and works with the team to adhere to care plans, ensuring equitable treatment and privacy.
10. Complies with corporate compliance and safety regulations.
11. Meets productivity standards and participates in surveys, in-services, and meetings.
12. Maintains current licenses, certifications, and records as per regulations.
13. Manages time effectively, prioritizes tasks, and meets outcomes and deadlines.

Minimum Education Required

• Nursing degree from an accredited college or university or graduate of an approved RN program.

Minimum Experience Required

• At least one (1) year of RN experience.
• Valid driver’s license.
• Reliable transportation.
• Liability insurance for home visits.

Minimum Licensure/Certification Required By Law

• Active, unrestricted RN licensure in the state of practice.
• Current CPR certification.
