Job summary

Corewell Health West, Visiting Nurses Team, seeks experienced RNs for home health care in the West-Central Michigan area, including Grand Rapids. You will work autonomously to execute care plans and provide high-quality nursing services in patients' homes.

Eligible RNs must hold MI licensure, with a Bachelors in Nursing within 5 years, and are supported by a comprehensive benefits package and a $10,000 sign-on bonus for this position. Scheduling includes days and a 40-hour work week.

  • Bachelors in Nursing Required within 5 years.
  • MI Registered Nurse (RN) Required.
  • MI Driver’s License Required.
  • AHA or ARC Basic Life Support (BLS) Required within 90 days.

Responsibilities

  • Provide home health nursing under Nursing Manager supervision and collaborate with the interdisciplinary team.
  • Assess, monitor, and implement the treatment plan with clinical judgment to prevent complications.
  • Deliver patient care across the West-Central Michigan area, visiting 5–7 patients per shift.

About the unit

Our Visiting Nurses Association is our Home Care Skilled Nursing Team for West-Central areas of Michigan. We are accepting applications from experienced RN’s, LPN's, and New Grads! For experienced RN’s this is an autonomous position that drives you to use all of your top nursing skills and critical thinking. Working in home health enables you to treat the patient in the comfort of their home, where patients are able to thrive and own their own health care. We create personalized assignments close to home that enable our nurses to visit between 5-7 patients per shift. From Ludington, to Baldwin, to Nunica and south through Kent County, Ottawa County and as far south as Gun Lake, our Corewell Health West, Visiting Nurses Team goes the distance to provide outstanding patient care.

Scope of work

Under the direct supervision of the Nursing Manager, and in collaboration with the interdisciplinary team, the Nurse uses clinical judgment skills to access and implement the treatment plan for patients. The RN assesses monitors, detects, plans and takes appropriate action to prevent physiological complications associated with specific health problems and/or medical treatment.
