BrightStar Care is expanding in the St. Paul area and is seeking a Registered Nurse (RN) to serve as a Home Care Case Manager four days per week. This salaried role works closely with a team of RNs and LPNs to complete assessments, reassessments, and select skilled nursing tasks for private‑pay and veteran clients.

• Active RN license in Minnesota
• Experience in home health or acute care is advantageous
• Exceptional interpersonal and communication skills

Responsibilities

• Assess, plan, implement, and evaluate nursing care for patients in accordance with professional standards
• Collaborate with multidisciplinary team members to optimize patient care and outcomes
• Educate patients and their families on health maintenance and disease prevention
• Ensure accurate documentation and compliance with regulatory guidelines
