Job summary

BrightStar Care is seeking a dedicated Part Time RN Case Manager to provide in-home infusion and injection services in Minneapolis and surrounding areas. The role requires availability 3 days per week (8:00 AM–5:00 PM) with travel within a 30-mile radius and caring for 4–5 clients per day.

Compensation is visit-based ($70 for 1 hour; $50 for the first two hours; $45 thereafter), with mileage pay possible on a case-by-case basis. Join our infusion team today.

Qualifications

• Active RN license in Minnesota.
• Experience in a home health or acute care setting is advantageous.
• Excellent interpersonal and communication skills.

Responsibilities

• Assess, plan, implement, and evaluate nursing care for patients.
• Collaborate with multidisciplinary teams, pharmacies, and providers to optimize care.
• Educate patients and families on health maintenance and disease prevention.
• Ensure accurate documentation and regulatory compliance.
