Job summary

ComForCare Home Care is seeking an experienced Registered Nurse to conduct client assessments, create personalized care plans, and coordinate referrals. You will administer medications when necessary and maintain detailed records while helping clients live independently at home.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current RN license with the ability to practice in Minnesota.
• BLS certification.
• Demonstrated knowledge of safety and infection control practices.
• Ability to pass company background checks and drug screening.
• Valid driver's license with reliable transportation.

Responsibilities

• Perform comprehensive assessments of client status including physical, psychosocial, and environmental parameters.
• Develop an individualized plan of care for each client that incorporates data analysis and current findings.
• Provide interventions focused on achieving realistic client outcomes.
• Report changes to update the plan of care as progress is made.
• Maintain detailed client records showing assessments, planning, interventions, and evaluations.
• Initiate referrals to other health team members as needed.

What we are looking for:

• Graduate of an accredited school of professional nursing

Compensation: $40.00 - $50.00 per hour
