Job Description: Home Care Primary Nurse - Part Time

NeighborHealth, formerly East Boston Neighborhood Health Center, is one of the largest community health centers in the country, supporting the greater Boston area with a focus on patient and community well-being. We invite qualified candidates to apply for the Home Care Primary Nurse position in the PACE Community Care department.

Location: 155 Addison Street

Time Type: Part time

Position Summary: Nursing

The Home Care Primary Nurse oversees assessment, planning, and delivery of nursing care to participants, delegating tasks as appropriate while ensuring compliance with care plans and medical orders. Responsibilities include providing direct patient care in the PACE Day Health Center or in participants' homes, documenting care, and collaborating with the interdisciplinary team.

Essential Duties and Responsibilities:

• Assess patients by obtaining history, performing tests, observing participants and caregivers, and consulting with the team.
• Prepare patients for rapid provider review and treatment.
• Implement care plan interventions, testing, treatments, and provider orders.
• Oversee delegated tasks, monitor outcomes, and adjust care plans as needed.
• Administer medications, tests, and treatments safely and effectively, managing medication refills with provider authorization.
• Educate patients and families on health care regimens, disease management, and wellness topics.

Qualifications and Requirements:

• Baccalaureate degree in nursing preferred.
• Current Massachusetts nursing license.
• Current BLS certification.
• Minimum of 2 years experience in ambulatory, home health, hospital, or sub-acute settings preferred; equivalent training or experience considered.
• At least one year experience working with frail or elderly populations.

Pay Range: Starting at $40.00 up to $60.00 depending on experience.
