Position Details

Per Diem Home Care - RN Case Manager

Location: Northern Rhode Island

Schedule: Per Visit Pay Rate, Monday - Friday

Role Responsibilities

The RN Case Manager provides compassionate, direct patient care in clients' homes across Rhode Island. Responsibilities include assessment, education, and day-to-day nursing activities in a one-on-one setting. The role involves working with a collaborative interdisciplinary team, equipped with point of care devices and necessary medical supplies.

Qualifications

• Current and valid RN license in Rhode Island
• Preferred: home care experience
• Current CPR certification
• Reliable transportation, valid driver’s license, automobile insurance
