Heritage Hills Rehabilitation & Healthcare Center is seeking an RN for full-time and part-time roles in Rhode Island. The RN will provide skilled nursing care, monitor residents, and ensure care plans are followed with accuracy and compassion.

Qualifications

• Current, unencumbered Rhode Island RN license.
• Valid BLS (CPR) certification.
• Ability to manage multiple residents in a fast-paced setting.
• Strong clinical assessment, communication, and documentation skills.

Responsibilities

• Provide resident care per physician's plans and written care plans.
• Follow and implement care plans developed by Nurse Manager.
• Administer medications and treatments per plan of care.
• Monitor resident conditions and report changes promptly to clinical team.
• Maintain accurate documentation per physician orders, policies, and standards.
• Collaborate with interdisciplinary teams for coordinated care.
• Ensure infection control and safety compliance.
