Job summary

H. T. PROF Group in Rockford, IL seeks a Hemodialysis RN to deliver patient-centered chronic dialysis care in a long-term care setting under the Unit Medical Director.

Qualifications

• Graduate from an accredited school of nursing; BSN preferred.
• Approximately one year nursing experience; ICU/CCU experience preferred.
• Successful completion of training in theory and practice of hemodialysis.
• CPR Healthcare Recognition current.

Responsibilities

• TREATMENT MANAGEMENT: safely initiate, monitor, and discontinue hemodialysis; conduct pre- and post-treatment assessments.
• SPECIALIZED PROCEDURES: vascular access care, administer medications, draw blood, manage transfusions within scope.
• INFECTION CONTROL: follow isolation protocols and universal precautions.
• CARE PLANNING & EDUCATION: develop nursing care plans and educate patients and families.
• EMERGENCY RESPONSE: identify and report acute dialysis-related complications.
• EQUIPMENT OVERSIGHT: operate and maintain dialysis machines and water treatment systems.
• UNIT OPS: manage inventory, order supplies, maintain audit-ready workspace.
• TRoubleshooting: report equipment malfunctions.
• MULTIDISCIPLINARY COLLABORATION: coordinate with physicians, care coordinators, and staff.

About the Role

We are seeking a dedicated Hemodialysis RN to deliver exceptional, patient-centered chronic dialysis care within our long-term care facility. Working under the guidance of the Unit Medical Director, you will provide specialized bedside care, ensure strict adherence to state and federal standards, and build meaningful relationships with long-term residents.

Key Responsibilities

Clinical & Patient Care

Treatment Management: Safely initiate, monitor, and discontinue hemodialysis procedures; conduct thorough pre- and post-treatment clinical assessments.

Specialized Procedures: Perform vascular access care, administer medications, draw blood samples, and manage blood transfusions within your scope of practice.

Infection Control: Execute strict isolation protocols, universal precautions, and safe patient-handling techniques.

Care Planning & Education: Develop individualized nursing care plans and provide ongoing clinical education to patients and their families.

Emergency Response: Promptly identify, intervene, and report acute medical or dialysis-related complications.

Operations & Technical Management

Equipment Oversight: Safely operate, sanitize, and perform routine maintenance on hemodialysis machines and water treatment systems.

Unit Operations: Manage clinical inventory, order supplies, and maintain an organized, audit-ready clinical workspace.

Troubleshooting: Identify and immediately report equipment malfunctions or technical anomalies.

Multidisciplinary Collaboration

Interdisciplinary Care: Coordinate closely with physicians, medical directors, care coordinators, and facility staff.

Leadership & Mentorship: Assist with onboarding and training new clinical personnel; actively participate in Continuous Quality Improvement (CQI) initiatives and team meetings.

Requirements

• Knowledge of ESRD and hemodialysis procedures
• Commitment to ongoing clinical education and professional development

RN will be responsible for maintaining a working knowledge of all hemodialysis and related ancillary and emergency equipment utilized in the facility such as: reuse equipment, water treatment equipment, computer, hemodialysis machines, sphygmomanometer, conductivity meter, glucometers, scale, electronic thermometers, centrifuge, EKG machine, IV infusion pump, code cart, suction machine, oxygen administration equipment, and personal protective equipment.

MINIMUM QUALIFICATIONS

1. Graduate from an accredited school of nursing. Bachelors in nursing or health

related field preferred.

2. Approximately one year nursing experience.

5. ICU/CCU experience preferred.

6. Successful completion of training course in theory and practice of hemodialysis.

7. Valid driver’s license and insurance, when indicated/applicable for business

travel by automobile.

9. Successful completion of current CPR Healthcare Recognition according to the
