Avamere is seeking a Director of Health Services (RN) in Hillsboro, OR, full-time, day shift. Lead health services in a senior living facility, ensure regulatory compliance, coordinate care, and mentor the health services team.

Experience in ALF/Memory Care, CMS/state regulations, and EMR documentation is essential. The role requires strong clinical skills, leadership, and a passion for seniors in long-term care.

Qualifikationen

• Must possess a nursing degree from an accredited college or university.
• Must possess a current, unencumbered, active RN license in Oregon (BSN preferred).
• Nursing experience in assisted living, memory care, or geriatric care in long-term settings.
• Thorough knowledge of CMS and state regulations governing patient care in ALF/IL/MC.
• Experience with electronic medical records documentation.
• Strong leadership, communication, and staff education skills.
• Ability to design and deliver effective training programs.
• Excellent organizational and time management skills.
• Maintain CPR/BLS certification during employment.
• Maintain confidentiality of resident care information.
• Desire to improve quality of life for seniors.

Aufgaben

• Conduct resident assessments and update care plans.
• Develop and evaluate person-centered care plans with residents and families.
• Ensure regulatory compliance and participate in audits.
• Oversee medication management and audits for compliance.
• Provide leadership, mentorship, and education to care staff.
• Foster open communication with residents, families, and providers.
• Coordinate with physicians, therapists, and hospitals for care continuity.
• Apply critical thinking in emergencies and evolving health situations.
• Participate in quality improvement initiatives and IDT meetings.
• Participate in on-call rotation after hours and weekends.
