Job summary

Sapphire Health Services, LLC is seeking a Director of Health Services to oversee clinical operations for an Assisted Living and Memory Care community in Clatskanie, OR. The DHS will lead nursing staff, ensure regulatory compliance, and uphold high-quality resident care and safety.

Qualifications

• RN licensure in the state of practice (Oregon).
• Minimum of two years of nursing leadership experience preferred.
• Experience in Assisted Living, Memory Care, Skilled Nursing, or geriatric care preferred.

Responsibilities

• Participate in infection prevention and control activities.
• Ensure medication management practices meet regulatory standards.
• Train, coach, and supervise nursing and clinical staff.
• Provide ongoing education and competency evaluations.
• Conduct performance evaluations and corrective action when appropriate.
• Promote employee engagement, teamwork, and professional development.
• Assist with staffing plans and scheduling to ensure adequate clinical coverage.
• Build trusting relationships with residents and their families.
• Communicate changes in resident condition promptly.
• Perform and oversee nurse delegation in accordance with applicable state regulations, including resident assessments, caregiver training, competency validation, and ongoing supervision.
• Address resident and family concerns professionally and compassionately.
• Oversee medication administration systems.
• Ensure medications are ordered, stored, administered, and documented appropriately.
• Collaborate with consulting pharmacists.
• Monitor medication passes and provide coaching as needed.
• Partner with the Administrator/Executive Director to support overall community operations.
• Collaborate with Life Enrichment, Dining Services, Maintenance, Housekeeping, and other departments.
• Foster a positive, resident-centered culture.
• Respond to medical emergencies and critical incidents.
• Participate in emergency preparedness planning and drills.
• Serve as the clinical leader during emergency situations.
• Other duties as assigned.
