Qualifications

• BSN-prepared registered nurse.

Responsibilities

• Coordinate comprehensive patient care for acute, chronic, complex medical and social needs.
• Review medical records, labs, referrals to address follow-up needs.
• Coordinate referrals, testing, medications and communications among care team.
• Oversee daily clinical operations and patient flow with leadership.
• Supervise and mentor clinical staff; develop workflows and protocols.
• Monitor quality measures and support population health initiatives.
• Ensure HIPAA and safety compliance; maintain documentation in the EHR.
