Job summary

Riverside Health System is seeking a Registered Nurse in Gloucester, Virginia to oversee the total nursing care of assigned patients. Under the supervision of the Nurse Manager and Director, you will assess, plan, implement, and evaluate care using evidence-based practices and culturally competent approaches.

You will lead a care team, educate patients and families, document per RHS standards, and coordinate transitions to higher levels of care while maintaining safety and quality outcomes.

Qualifications

• Education: Nursing Diploma or higher; BSN/ADN preferred as applicable.
• Licenses: RN license with compact state eligibility upon hire (Virginia).
• CPR/BLS certification required upon hire.

Responsibilities

• Assess patient clinical and psychosocial needs and plan care.
• Develop, implement and evaluate individualized care plans with the team.
• Coordinate care with interdisciplinary team to meet patient outcomes.
• Document accurately per RHS standards and regulatory requirements.

Gloucester, Virginia Hiring Range $36.50 - $50.23/Hourly Actual pay is determined based on job-related factors such as relevant experience, education, credentials, skills, internal equity, and business needs.

Overview Works under the supervision of the Nurse Manager and the Director and is responsible for the assessment, planning, implementing, and evaluating the total nursing care of assigned patients. The RN utilizes communication, critical thinking, problem solving, and decision making to create a care plan for each patient based on physiological, psychosocial and cultural diversity. Responsible for supervising and delegating non-professional personnel working directly with him/her to ensure optimum patient care. Must demonstrate competence in dealing with age related needs of the specific patient populations served in nursing unit. Works under the direction of leadership and a multi-disciplinary team to provide a comprehensive range of services or clinical care associated with and in accordance with standards of excellence established by Riverside Health system and facility goals and strategic plans.

• Demonstrates nursing unit specific competencies and equipment training, see checklists.
• Partners with patients and their family to develop, implement and evaluate a plan of care, treatments and procedures that contribute to physiological and psychosocial outcomes of the patient.
• Incorporates evidence based standards of practice and collaborates with interdisciplinary team in care coordination.
• Facilitates safe patient transition to next level of care as part of plan of care.
• Demonstrates sense of urgency to facilitate patient throughput to ensure patient receives consistent, competent and ethical care.
• Focuses on quality outcomes of patient when addressing service needs to ensure evidence based quality care of patient.
• Demonstrates understanding of diverse patient care populations.
• Modifies education method to ensure patient/family understanding.
• Assumes responsibility to coordinate and deliver individualized patient education and discharge specific education to family/caregivers.
• Evaluates for comprehension and documents.
• Accurately completes all required clinical and quality documentation consistently per RHS standards and regulatory requirements.
• Delegates and prioritizes tasks appropriately to facilitate quality patient care and service excellence to meet the patient's care needs.
• Recognizes and responds to emergency situations.

Qualifications

• Education Bachelors Degree, Nursing (Preferred) or Associates Degree, Nursing (Preferred) or Program Graduate, Nursing (Nursing Diploma is minimum education requirement, Associate’s or Bachelor’s in Nursing accepted in lieu of Nursing Diploma) (Required) and High School Diploma or GED, HS Diploma/GED only required if Nursing Diploma Graduate (Required).
• Licenses and Certifications Registered Nurse (RN) - Virginia Department of Health Professions (VDHP) or licensed in a compact state Upon Hire(Required) and CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) Upon Hire(Required).
