Job summary

Chapters Living in Illinois is seeking a dedicated Registered Nurse to join our team. You will assess residents, develop and implement care plans, monitor health, administer medications, and collaborate with physicians and the wellness team to provide compassionate, person-centered care.

The role requires an active RN license and at least 1 year of experience in healthcare or long-term care, with strong communication skills and the ability to work both independently and in a team.

Qualifications

• Active RN license in the state of practice.
• Minimum 1 year of RN experience in a healthcare setting.
• Experience in assisted living or long-term care preferred.
• Strong communication and interpersonal skills.
• Ability to work independently as well as in a team.
• Excellent organizational and time management skills.
• Basic computer skills and EMR experience.

Responsibilities

• Assess residents' health needs, develop and implement care plans, and evaluate effectiveness.
• Administer medications and treatments as prescribed by physicians.
• Provide wound/ catheter care and other nursing interventions as needed.
• Monitor and document vital signs and medical history.
• Collaborate with physicians, nurse practitioners, and social workers to provide comprehensive care.
• Educate residents and families on health and disease management.
• Participate in staff meetings and ongoing training.
