Job summary

Alameda Health System is seeking an AHD Registered Nurse for Park Bridge in Alameda, CA. The nurse provides direct patient care, supervises CNAs/LVNs, and ensures care aligned with state and federal standards. Hours may vary as the unit operates 24/7.

Responsibilities include coordinating admissions/discharges, conducting rounds, documenting assessments, and collaborating with physicians to ensure safe, effective care. CNAs and LVNs are supported with ongoing supervision and training.

Qualifications

• Graduate of an accredited Nursing program required.
• Bachelor's degree preferred.
• Minimum one year of directly related experience; new graduates considered.
• Valid California RN license and current BLS certification.

Responsibilities

• Provide direct nursing care to Residents, following current standards and policies.
• Coordinate admission, discharge, and transfer of residents and monitor rounds every two hours.
• Assess, document and report residents' health status and responses to care.
• Consult with physicians on orders and ensure safe medication administration.
• Review medication/treatment records for accuracy and policy adherence.
• Educate and counsel residents and families about health needs and options.
• Plan and document restorative/nursing interventions to promote independence.
• Supervise CNAs/LVNs, teaching nursing rehabilitation techniques.
• Collaborate with therapies (PT/OT/ST/RT) to coordinate care plans.
