Brooks Rehabilitation in Fernandina Beach, FL is seeking a full-time Registered Nurse to manage patient care in collaboration with a multidisciplinary team. The role follows the Florida Nurse Practice Act and includes planning, documentation, and discharge teaching.

Responsibilities include developing care plans within 24 hours of admission, coordinating with families and rehab teams to promote independence, and participating in team meetings.

Qualifications

• Active Florida RN license required.
• BLS certification required.
• Associate, Bachelor’s, or Master’s degree in Nursing from an accredited school.

Responsabilités

• Identify actual/potential nursing/clinical problems from assessment data and document findings.
• Develop a comprehensive nursing plan of care within 24 hours of admission.
• Collaborate with clients, families and rehab team to set patient-centered goals.
• Document changes and follow up with clinical team.
• Provide independent nursing care in emergencies and act as a resource to the team.
• Complete medication reconciliation at each SN visit and monitor reactions.
• Deliver teaching and discharge planning for patients and families.
• Collaborate with interdisciplinary team and participate in team meetings.
