Job summary

Enhabit Home Health & Hospice seeks a compassionate Registered Nurse to provide care in home health and hospice settings. You will coordinate patient care with families and health professionals, perform OASIS data collection, and monitor medications and lab values to keep physicians informed.

This hybrid role offers diverse clinical experience across home health and hospice, enabling you to support patients and families through their full care journey while maintaining high standards of care.

Qualifications

• Must be a graduate of an approved school of nursing and licensed as a Registered Nurse in the state of employment.
• Minimum of one year of clinical experience is preferred.
• Previous home health or hospice experience is preferred.

Responsibilities

• Perform visits and coordinate patient care in home health and hospice settings.
• Oversee frequency of visits and coordinate with physicians and the office.
• Accurately perform OASIS data collection and ensure medication profiles are current.
• Monitor lab values and report changes to physicians.
• Communicate compassionately with patients and families and maintain high ethical standards.
