Job summary

Enhabit Home Health & Hospice in the United States is seeking a dedicated Registered Nurse to join our home‑based care team. You will coordinate care, assess patient and family needs, and implement the interdisciplinary care plan.

Must be a graduate of an approved nursing program, licensed in the state where you practice, and have at least one year of RN experience. You should also be comfortable using mobile devices; Medicare home health or hospice experience is preferred.

Qualifications

• Must be a graduate of an approved school of nursing education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed registered nurse (RN).
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Provide coordination of care, ensure continuous assessment of each patient's and family's needs.
• Implement the interdisciplinary care plan and ensure delivery of care is compliant with orders and regulations.
