Ottawa County Community Mental Health is seeking a full-time nurse with case management experience to provide health assessments, counseling, and direct care for developmentally disabled consumers in Holland, MI. Qualifications

• Bachelor of Science in Nursing or RN with 3+ years of experience in nursing services for the developmentally disabled
• Valid State of Michigan Registered Nurse license
• Strong knowledge of medical/surgical nursing and health care regulations (HIPAA, Medicaid, CARF)
• Able to coordinate care across home/community settings and with guardians or caregivers
• Ability to document accurately and maintain privacy of health information

Responsibilities

• Provide nursing and case management services to developmentally disabled individuals in homes and community settings
• Interview and counsel consumers, caregivers, families, and associated providers
• Perform initial nursing assessments, health screenings, and administer medications and treatments
• Monitor health problems and adjust care plans with the team
• Develop and revise plans of nursing care with caregivers; ensure follow-up services
• Educate consumers and caregivers about treatment plans and medications
• Collaborate with supports coordinators and clinicians on treatment plans
• Maintain compliance with regulatory and accreditation standards
• Maintain case documentation and protect patient health information
