Plaza Health and Rehabilitation in Charlotte, NC is seeking Registered Nurses (RNs) to provide resident-centered care in a skilled nursing and long-term care setting. Full-Time, Part-Time shifts available.

You will administer medications, coordinate with therapists, and document care according to state and facility policies. A strong RN license and CPR/BLS are required, with opportunities for advancement and comprehensive benefits.

Qualifications

• Active RN license required.
• CPR/BLS Certification required.
• Experience in Long Term Care or Skilled Nursing preferred.
• Excellent technical, assessment and documentation skills.
• Strong analytical and problem-solving skills.
• Ability to lift 25+ pounds.
• Ability to stand, bend, and walk for extended time periods.
• Team player, strong communicator, passionate about senior care.

Responsibilities

• Provide nursing care to meet resident needs per standards, orders, policies and regulations.
• Plan nursing interventions for each resident and monitor implementation.
• Evaluate care interventions, identify problems and develop alternatives.
• Conduct family and resident education.
• Administer medications and treatments per policy.
• Transcribe orders and document progress notes accurately.
• Identify changes in condition and report to supervisor, physician and family.
• Coordinate with therapists to ensure rehab needs are met.
