Responsibilities

• Collaborate with the healthcare team to assess, plan, implement, and evaluate individualized resident care plans.
• Administer medications (oral, subcutaneous, and IV) and document accurately while monitoring for therapeutic response and adverse reactions.
• Provide direct bedside care to residents with a wide range of medical needs, including pre- and post-surgical care.
• Monitor residents' clinical status and manage or adjust specialized equipment as needed.
• Recognize and respond promptly to changes in resident condition and initiate appropriate interventions.
• Perform wound care, catheter care, and IV starts as needed within scope of practice.
• Educate residents and families regarding procedures, treatment plans, and ongoing care.
• Maintain accurate, timely, and complete documentation of all nursing care.
• Participate in discharge planning and care coordination with the interdisciplinary team.
