Job summary

Ceres PostAcute Care is seeking a Registered Nurse in California to provide direct and indirect patient care, develop nursing care plans, and work collaboratively with physicians and the care team. The role involves documenting care, administering IV medications, and supervising LVNs and CNAs.

Qualifications

Requires active RN license in California.
Current CPR certification.
Experience supervising LVNs and CNAs.
Strong observation, problem-solving, and communication skills.
Ability to work with minimal supervision.

Responsibilities

Provide planning and delivery of direct and indirect patient care through the nursing process (Assessment, Planning, Intervention, Evaluation).
Develop nursing care plans in coordination with patient, family, and interdisciplinary staff.
Communicate changes in patient’s condition with Physicians, Nursing Supervisor/Manager, and team.
Document assessments and care in compliance with standards and company policy.
Administer IV medications and treatments.
Collaborate with facility interdisciplinary team.
Supervise care provided by LVNs and CNAs.
Carry out other duties as assigned.
Manage and participate in AM, PM, and NOC shift rotations.
