Qualifications

• Licensed RN in the state of practice.
• Must hold and maintain a current CPR certification.
• License in good standing; no finding of abuse, neglect, or misappropriation in registry.

Responsibilities

• Assure that effective quality nursing care is delivered and outcomes-focused.
• Identify needs and input for discharge planning and follow-up.
• Collaborate with resident, family, and interdisciplinary care team to develop plan of care.
• Implement plan of care consistently and cost-efficiently with resident-centered outcomes.
• Share information with physician and other health care professionals to adapt the care plan.
• Deliver and maintain optimum resident care and comfort with current nursing practices.
