Qualifications

• Must hold a current RN license in state of practice.

Responsibilities

• Deliver quality nursing care focused on outcomes using the nursing process.
• Assist with discharge planning and follow-up across length of stay.
• Develop individualized care plans with residents and care team.
• Implement plans effectively with resident-centered outcomes and available resources.
• Collaborate with physicians and other health care professionals and share relevant information.
• Deliver and maintain optimal resident care and comfort with current nursing practices.
