Formación

• Registered nursing license in IL.
• BSN degree.
• BLS certification from AHA.
• National Certification in specialty (preferred).

Responsabilidades

• Performs a comprehensive assessment on all assigned patients and documents outcomes.
• Communicates changes in patient condition promptly.
• Uses nursing diagnosis and assessment data to set measurable goals in the care plan.
• Identifies patient needs and prioritizes nursing diagnoses and documents.
• Collaborates with patient, family, and health care team; plans discharge and makes referrals as needed.
• Evaluates patient response to interventions and updates the care plan accordingly.
• Discharges planning and provides education and medication reconciliation at discharge.

Educación

Registered Nursing license issued by the State of Illinois

Bachelor of Science in Nursing

BLS through American Heart Association

National Certification in area of specialty
