Job summary

Adaptive Home Health in Detroit is seeking a Field Supervisor (RN) to lead and mentor clinicians in a growing home health program. This salaried, exempt role blends patient care with teaching and leadership.

You will precept new hires, validate competencies, and support ongoing education at the Southfield branch, with a focus on OASIS accuracy and CoP adherence. Expect field-based duties, Monday–Friday, with mileage reimbursement.

Qualifications

  • Active, unrestricted Michigan RN license.
  • Minimum 2 years of home health experience.
  • Clean disciplinary record with no adverse action in last 12 months.
  • Current Michigan driver's license and field-based work comfort.
  • Strong clinical assessment skills with OASIS knowledge.
  • Ability to coach and provide constructive feedback.

Schedule: Full-time Monday-Friday
Location: Field-based, Detroit metro. Branch office in Southfield, MI
Reports to: Clinical Manager / Branch Director / Director of Clinical Education

Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health model across the US, and we launched our Detroit branch this year. Our ultimate mission is to dramatically improve patient access to home health care.

The Field Supervisor role is how we make that mission scale: you guide newly hired and transitioning clinicians through real-world home health practice, validate their clinical competency, and model the standard of care every patient deserves.

Who We Are

We build technology to better support our field clinicians - faster and more supported. Our clinicians are the customers of our engineering team. Our charting product is designed to reduce documentation burden and keep visits moving, and our scheduling product (launching soon) will make it easier to coordinate visits, reduce last-minute changes, and protect clinician time.

For Field Supervisors specifically, you'll have direct input into how those tools evolve through Adaptive focus groups - your frontline experience shapes the product roadmap.

If you have strong clinical judgment, love developing other clinicians, and want autonomy with real organizational impact, this role gives you a meaningful blend of patient care, teaching, and product influence.

What You Will Do
  • Time allocation: 75% field work (independently or with new joiners), 15% dedicated education time at your local branch, 10% special projects (e.g., Adaptive focus groups, improving education resources)

  • Precept and coach newly hired clinicians during patient visits, providing real-time clinical guidance and feedback

  • Provide hands-on mentorship and support to new joiners - with particular focus on clinicians with no prior home health experience

  • Evaluate and sign off on new joiners' clinical competency through structured observation during patient visits

  • Assist with skills lab training sessions and other structured onboarding activities

  • Administer and support end-of-orientation assessments for new hires

  • Serve as a resource and primary point of contact for new joiners throughout their orientation period

  • Maintain a reduced but active patient caseload to stay current with clinical practice while dedicating meaningful time to precepting and teaching

  • Maintain quality standards, including <15% missed visit rate

  • Model best practices in OASIS documentation, clinical decision-making, and adherence to Conditions of Participation (CoP)

  • Identify and elevate concerns related to new joiner readiness or patient safety to the Clinical Manager

  • Participate in Adaptive improvement focus groups, contributing frontline clinical insight to refine and enhance Adaptive's tools and workflows

Requirements

Must-haves

  • Active, unrestricted Michigan RN license.

  • Minimum 2 years of home health experience (current or prior employer)

  • Clean disciplinary record - no adverse action in the last 12 months

  • No licensure encumbrances within the last 5 years

  • Current CPR/BLS certification

  • Current Michigan driver's license and comfort with field-based work

  • Strong clinical assessment skills and OASIS knowledge

  • Patient, encouraging communication style with the ability to give constructive feedback

  • Paraprofessionals (LPN) are not eligible

Nice-to-haves
  • Prior experience as a preceptor, clinical mentor, or educator in any setting

  • COS-C certification (or willingness to obtain - we cover the cost)

  • Familiarity with Homecare Homebase (HCHB) and PointCare

  • OASIS-E2 trained, HCS-D, or wound care certification

  • Experience training clinicians who are new to home health
