Job summary

Beth Israel Lahey Health in Boston is seeking a Command Center Staff RN to deliver high-quality patient care within the Hospital at Home model, reporting to the Hospital at Home Director of Nursing.

You will perform protocol-driven assessments, monitor biometric data, and coordinate care across oncology, medicine, and post-operative populations, while collaborating with a multidisciplinary team to ensure safe, evidence-based practices.

Qualifications

  • Nursing Diploma or ASN, BSN preferred.
  • A current Massachusetts RN license is required.
  • BLS certification is required.
  • Three years RN experience in clinical settings.

Responsibilities

  • Provide protocol-driven assessment, care coordination and interventions.
  • Care for diverse patient populations including oncology, medicine and post-op.
  • Maintain nursing guidelines per state and hospital regulations.
  • Facilitate hospital at home admissions per guidelines.
  • Collaborate with multidisciplinary team and mentor staff.
  • Support quality improvement and evidence-based practice.
  • Perform other duties as assigned by senior leadership.

Reporting directly to the Hospital at Home Director of Nursing, The Command Center Staff RN is responsible for providing high-quality patient care to various patient populations within the framework of the Hospital at Home model.

Job Description
Essential Duties & Responsibilities including but not limited to:
  • Utilizing monitoring, assessment, planning, intervention and critical thinking skills to care for acute care patient populations in the home
  • Navigating patients and families across the continuum of care
  • Monitoring and responding to biometric data per Hospital at Home Command Center protocols
  • Conducting audio/visual patient care visits as directed by the care team and/or protocol
  • Ensuring all interventions in the patient’s care plan are implemented efficiently with a patient centered approach
  • Leveraging the resources and expertise of supplier partners to implement collaborative workflows, standards, policies, protocols, guidelines and documentation systems to support safe, reliable, high-quality, evidence-based care with clinical protocols as the foundation
  • Adhering to the clinical protocols and be willing to obtain the necessary training to provide care within the context of the providing evidence-based care
  • Promoting professional practice and a culture of safety; willingness to engage in process improvement efforts
Clinical Roles And Responsibilities
  • Providing protocol driven assessment, care coordination and interventions as appropriate informed by Hospital at Home protocols for diverse patient populations with a focus on oncology, medicine and post- operative care
  • Exhibiting sound clinical decision making and critical thinking skills
  • Maintain and practice in accordance with nursing guidelines per state and hospital regulations
  • Facilitating admissions to l Hospital at Home utilizing Hospital at Home guidelines
  • Serving as a role model for professional nursing practice
  • Utilizing nursing knowledge to identify, prevent and/or solve complex acute and/or recurring patient care problems
  • Sharing expertise with the multidisciplinary team and serve as a resource to staff in area(s) of expertise
  • Supporting research efforts to advance knowledge and promote evidence-based practice
  • Preforming other duties and responsibilities as assigned by the Senior Director of Nursing
Quality And Safety Responsibilities
  • Ensure compliance with reporting requirements for Sentinel Events and Serious Incidents
  • Participate in quality and safety investigations and root cause analyses
  • Take appropriate action to ensure patient safety as a result of any investigation; and
  • Serve as a resource in quality improvement processes and technique

Education
Minimum Qualifications:

Nursing Diploma or ASN, BSN preferred

Licensure, Certification, Registration
  • A current Massachusetts RN license
  • BLS certification

Hours of work may be subject to change based upon the needs of the business/department.

Pay Range

$37.00 - $95.74

The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
