Job summary

Longevity Health is seeking an RN Field Coordinator to blend hands-on patient care with administrative coordination in long-term care settings. You will travel to nursing homes, support Advanced Practice Providers, and help with care transitions and documentation.

Requirements include an Associate degree in nursing, a RN license, at least two years of RN experience, and willingness to travel. Knowledge of managed care coordination and EMR systems is a plus.

Qualifications

• RN license in good standing required.
• Associate degree in nursing required; higher degree preferred for licensed nurses.

Responsibilities

• Visit multiple nursing homes to assist APNs with clinical and admin tasks.
• Prescreen enrollees in EMRs and identify continuity of care needs.
• Collaborate with clinical and operations teams to oversee member appointments and care transitions.
• Support transitions of care, prior authorizations, and access to services outside the facility.
• Communicate with patients, families, and physicians as delegated by nurse practitioners.
• Lead orientation sessions for nursing homes on Longevity Health Plan model of care.
