Longevity Health is seeking an RN Field Coordinator to blend hands-on patient care with administrative coordination in long‑term care settings. The RN Field Coordinator will travel to various Nursing Homes upon request, supporting clinicians and educating facility staff on our clinical model to ensure smooth transitions of care.
Requirements include an active RN license, 2+ years’ nursing experience, strong verbal and written communication, EMR proficiency, and the ability to travel.
