The RN Clinical Care Coordinator will serve as the primary care manager for members with complex medical and behavioral needs. Responsibilities include supporting medical, behavioral, and socioeconomic needs to promote appropriate service utilization and improve care quality. If you reside in Worcester County, MA, you will have the flexibility to work remotely, with local travel up to 75% and mileage reimbursed.

Primary Responsibilities
  • Engage members face-to-face and/or via phone for comprehensive needs assessments.
  • Develop and implement person-centered care plans addressing health conditions, wellness, social determinants, medication management, and safety.
  • Collaborate with care teams, providers, and community resources to execute care plans.
  • Provide education and coaching for self-management and lifestyle changes.
  • Support discharge planning and coordinate care transitions post-ER visits, hospital stays, or SNFs.
  • Advocate for members to ensure their needs and preferences are supported.

Minimum Qualifications
  • Current, unrestricted RN license.
  • At least 2 years of clinical experience.
  • Proficiency with MS Office.
  • Dedicated, secure home workspace with PHI protection.
  • Reliable internet and transportation, with a valid US driver’s license.
Preferred Qualifications
  • Bachelor’s or Master’s in Nursing.
  • Care Manager certification (CCM).
  • Community case management experience.
  • Background in Managed Care and team-based care.
  • Experience with EMRs and virtual care platforms.
Physical Requirements
  • Ability to travel within the territory and visit various locations.
  • Transport equipment as needed.
  • Remain stationary for computer work.
  • Adhere to telecommuting policies.
