Role Overview

The RN HSS Clinical Care Coordinator plays a vital role in our Integrated Care Model by relaying member needs, advocating for optimal care, and ensuring access to appropriate services. If you reside in Worcester County, MA, and nearby areas, you will have the flexibility to work remotely while engaging in field visits.

This is a field-based role, requiring approximately 80% of your time visiting members at their homes or facilities. Flexibility, adaptability, and patience are essential qualities for success in this position.

Primary Responsibilities
  • Engage members face-to-face and via phone to conduct comprehensive needs assessments covering medical, behavioral, functional, cultural, and socioeconomic aspects.
  • Develop and implement individualized care strategies aimed at the least restrictive, most appropriate care levels.
  • Identify and refer members to social services, financial assistance, psychosocial support, and community resources.
  • Manage care plans throughout the care continuum, serving as the main point of contact.
  • Collaborate with care teams, providers, and community partners to execute care plans effectively.
  • Communicate health-related information clearly to all stakeholders to ensure coordinated, timely care.

Required Qualifications
  • Current, unrestricted RN license
  • At least 2 years of clinical experience
  • Proficiency in MS Office (Word, Excel, Outlook)
  • Dedicated, secure home workspace and PHI protection
  • High-speed internet access in your location
  • Reliable transportation and ability to travel within the region
  • Valid US driver’s license
Preferred Qualifications
  • Bachelor’s or Master’s in Nursing
  • Certified Care Manager (CCM)
  • Experience in community case management and managed care
  • Team-based care experience
  • Electronic Medical Record proficiency
  • Bilingual in Spanish

The salary range is $28.61 to $56.06 per hour, based on full-time employment and various factors. Benefits include comprehensive packages, incentives, and stock options.
