As a Field-Based RN Case Manager with UnitedHealth Group, you'll work in various environments, providing a sense of satisfaction by helping create an efficient health care system. This position is based in Southeast Houston, TX, with flexibility for hybrid/field work.

Primary Responsibilities:
  1. Provide comprehensive quality care through close communication with members via in-person or phone interactions.
  2. Support members with condition education, medication reviews, and connecting to resources such as Provider Services, Private Duty Nursing, DME, and supplies.
  3. Assist members transitioning from care facilities back to their homes.
  4. Travel approximately 75% of the time, working in various environments.
  5. Work autonomously, maintaining focus and motivation.
  6. This role is for UHC C&S only, addressing medical, behavioral, and social determinants of health to improve well-being.
  7. Work is self-directed, handling complex issues and serving as a resource to others.

Qualifications:
Required:
  • Associate's Degree or higher
  • Current RN license
  • 3+ years clinical experience
  • Computer proficiency
  • Reliable transportation & valid US driver’s license
Preferred:
  • Case management certification
  • Experience with vulnerable populations and home/community care
  • Bilingual skills

The salary range is $28.61 to $56.06 per hour, based on various factors. Benefits include a comprehensive package, incentives, stock purchase, and 401(k).
