RN Field Clinical Care Coordinator - Star Kids - Southeast Houston, TX

This is a field-based position with a home-based office, offering flexibility to work a hybrid/field schedule in the Southeast Houston, TX market. The role involves approximately 75% travel, working in various environments including in-home and facility-based settings.

Primary Responsibilities
  • Provide comprehensive quality care through close communication with members via in-person or phone interactions.
  • Support members with condition education, medication reviews, and connecting to resources such as Provider Services, Private Duty Nursing, DME, and supplies.
  • Assist members with transitioning from care facilities back to their homes.
  • Work autonomously, managing complex issues and serving as a resource to others.

Required Qualifications
  • Associate's degree or higher.
  • Current unrestricted RN license.
  • 3+ years of clinical experience in hospital, acute care, home health/hospice, direct care, or case management.
  • Intermediate computer proficiency for electronic records, email, internet, and software applications.
  • Reliable transportation and valid US driver's license.
Preferred Qualifications
  • Case management experience, certification preferred.
  • Experience with vulnerable populations with chronic or complex needs.
  • Experience in home & community-based long-term care services.
  • Bilingual skills.
Compensation

The salary range is $28.61 to $56.06 per hour, based on full-time employment, experience, education, and location. Benefits are comprehensive and include incentives, stock purchase options, and 401(k).
