AccordCare Home Health in Ormond Beach, FL is seeking an RN Field Case Manager for the Nonskilled Division to oversee non-skilled home care services. This primarily field-based role includes occasional in-office time to coordinate scheduling, authorizations, and client communications.

You will act as the primary liaison between clients, caregivers, and referral sources, ensuring high-quality care and strict compliance with state and federal regulations.

Qualifications

• Valid Florida RN license required.
• 1+ year of experience in home care or community health preferred.
• Excellent communication, leadership, and organizational skills.
• Reliable transportation and valid driver’s license for occasional field visits.
• Familiarity with AlayaCare software a plus.

Responsibilities

• Manage and coordinate non-skilled home care cases, ensuring clients receive exceptional service.
• Serve as the main liaison between clients, caregivers, and referral sources.
• Oversee daily office operations, scheduling, authorizations, and client communications.
• Conduct aide orientations, trainings, and supervisory visits to promote quality and consistency in care.
• Monitor compliance with AccordCare policies, as well as state and federal regulations.
• Collaborate with leadership to improve client satisfaction and caregiver performance.
