We currently have an opening for an experienced Home Health Care Registered Nurse. Experience with ICU, PICC Line, Wound Care, and Infusion is highly preferred.

Location and Area of Service

The corporate office is in Southfield, MI. The RN will support our service area within the Tri-County area (Oakland, Macomb & Wayne).

Job Responsibilities

• Provide services as per the plan of care
• Conduct initial and ongoing patient evaluations
• Develop and revise care plans
• Administer skilled nursing care and preventive procedures
• Document patient visits and care summaries timely
• Coordinate services and communicate patient needs
• Educate and counsel patients and families
• Participate in in-service programs and adhere to policies
• Process orders, notify physicians of changes, and complete certifications
• Refer patients to other specialists as needed
• Supervise and teach other nursing staff
• Participate in patient care conferences, peer review, and quality improvement
• Take on-call duties as scheduled
• Complete required assessments and documentation timely

Working Environment

Indoor work at agency office and patient homes, with travel within the service area.

Qualifications

• Graduate of an approved nursing program and licensed in Michigan
• At least one year of nursing experience
• Kinnser EMR experience preferred
• Home healthcare experience preferred
• Experience with wound vac, PICC line, mediport care preferred
• Pediatric experience is a plus
