Location and Coverage

Based in Southfield, MI, supporting the Tri-County area (Oakland, Macomb, Wayne).

Job Responsibilities

1. Provide skilled nursing care as per the plan of care.
2. Conduct initial and ongoing patient evaluations.
3. Develop and revise care plans.
4. Administer specialized nursing services.
5. Coordinate and document patient care activities.
6. Communicate patient needs and changes to the healthcare team.
7. Educate patients and families.
8. Participate in trainings, peer reviews, and performance improvement.
9. Handle on-call duties as scheduled.
10. Complete all required documentation and assessments in a timely manner.

Qualifications

1. Licensed in Michigan, graduate of an approved nursing program.
2. Minimum one year of nursing experience.
3. Preferred: Kinnser EMR, home healthcare, Wound Vac, PICC line, Mediport, pediatric experience.

Working Environment

Work involves indoor office settings and patient homes, with travel within the service area.
