RN, Field Care Manager, Maternity, L&D, Mother Baby

Humana Healthy Horizons in Ohio is seeking an RN Field Care Manager 2 with expertise in Maternity, L&D, and Mother Baby. The role involves assessing and evaluating members' needs to promote and maintain optimal wellness. The Field Care Manager connects members and families with appropriate resources for their care and wellbeing.
The RN, Field Care Manager Nurse 2 works in a Care Management (CM) or CM Plus capacity, depending on member needs. Assignments are varied and often require interpretation and independent decision-making.
Responsibilities:
- Perform full scope of care coordination activities for members who need care coordination and are not assigned to a Care Coordination Entity (e.g., Ohio RISE Plan, CME, or MCO).
- Serve as the single point of contact for care coordination.
- Ensure members receive comprehensive assessment, care planning, and ongoing needs management, coordinating with assigned entities as necessary.
- Employ strategies to manage physical, environmental, and psycho-social health issues, overcoming barriers to effective care.
- Use a holistic, member-centric approach to engage members and families in health and wellness programs.
- Conduct telephonic and face-to-face assessments to guide members toward appropriate resources.
- Develop care plans based on clinical judgment, health changes, and psychosocial factors.
- Collaborate with providers and community services to promote quality, cost-effective outcomes.
- Coordinate services/supports across the care team, including transitions of care.
- Understand organizational strategies and contribute to departmental objectives.
- Submit incident reports as required.
Required Qualifications:
- Must reside in Ohio.
- Active RN license in Ohio without restrictions.
- Minimum two years of clinical experience, including Maternity, L&D, and Mother Baby.
- Proficient in Microsoft Word, Outlook, Excel, and web navigation.
- Excellent communication and interpersonal skills.
- Ability to work independently within scope of practice.
- Compliance with Humana's TB screening and vaccination policies.
- Valid driver's license and proof of insurance with limits of 100,000/300,000/100,000.
- Home internet with minimum 25 Mbps download and 10 Mbps upload speeds; dedicated workspace.
Preferred Qualifications:
- Case Management Certification (CCM).
- Experience with Medicare, Medicaid, or dual-eligible populations.
- Field Case Management experience.
- Knowledge of community health and social services.
- Bilingual abilities (e.g., Spanish, Haitian Creole, Somali).
Additional Information:
- Work involves remote and field-based member visits, with up to 50% travel.
- Standard work hours are 8:00 AM to 5:00 PM EST, Monday through Friday.
- Travel to Humana offices for training or meetings may be required.
- Compensation ranges from $71,100 to $97,800 annually, plus potential bonuses.
