Job summary

LHC Group is seeking a PRN RN Field Assessor to perform Long Term Care insurance assessments in Greenwich, CT. You will gather claimant demographics, physician information, and current medications while evaluating functional independence and cognitive status.

You will document in real time using a point-of-care web application on a laptop or tablet, conduct brief cognitive screening, and assess ADL/IADL capabilities in home settings.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current CPR certification.
• Proven ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
• Current driver’s license, vehicle insurance and access to a dependable vehicle or public transportation.
• 1+ years of RN experience.
• Proven ability to work independently.
• Proven good communication, writing, and organizational skills.

Responsibilities

• Complete Long Term Care insurance assessments.
• Obtain claimant demographics, physician information, a current medication list, and evaluate the claimant’s functional level of independence.
• Complete a brief cognitive screening and evaluate the claimant’s ability to complete ADL and IADL tasks.
• Be technologically savvy and use a point-of-care web application/system on a laptop or tablet in real time for documentation purposes.
