Job summary

Comfort Keepers is seeking an energetic RN or LPN to serve as an External Care Coordinator, building relationships in the Charleston community and educating families on personalized homecare plans.

You will conduct in-home assessments, develop care plans, and partner with physicians and hospitals to ensure seniors remain safe and independent at home. Strong communication and self-motivation are essential.

Qualifications

• Active RN or LPN license.
• Graduate of an accredited nursing program.
• Minimum one year of nursing experience.
• Home care experience preferred.

Responsibilities

• Be the face of Comfort Keepers in the community.
• Develop lasting partnerships with physicians, hospitals, rehab centers, hospice providers, and other referral sources.
• Meet with families to assess needs and recommend care solutions.
• Complete in-home assessments and develop individualized care plans.
• Guide clients through every step of their journey with Comfort Keepers.
• Collaborate with internal team to ensure exceptional care.
