Job summary

Comfort Keepers is seeking a dynamic Registered Nurse (RN) to join our North Carolina team as a Care Coordinator (RN). You will build relationships, work in clients’ homes, and coordinate personalized care with families, physicians, and our team.

The ideal candidate is an organized, people-focused RN with home care experience who can travel regionally in NC and uphold our service plans and quality care standards.

Qualifications

• Current Registered Nurse license through the North Carolina Board of Nursing.
• Home care experience preferred.
• Reliable transportation and ability to travel throughout NC.
• Strong communication, organization, and relationship-building skills.
• Genuine passion for helping seniors and their families.
• Ability to meet background screening, driving record, and other employment requirements.

Responsibilities

• Conduct initial in-home evaluations for prospective clients and determine admission suitability.
• Develop, document, and initiate individualized client Service Plans.
• Provide hands-on training to caregivers/CNAs and PCAs.
• Conduct onsite supervisory visits for personal care aides, caregivers, and companion services.
• Re-evaluate client needs at least quarterly and update Service Plans as appropriate.
• Help coordinate care between clients, families, caregivers, physicians, and agency team members.
• Communicate changes in a client’s medical or mental condition to the appropriate physician and care team.
• Provide education and guidance to family and household members assisting with personal care.
• Participate in quality assurance reviews and ongoing evaluation of agency services.
