Job summary

Signature HealthCARE is seeking a seasoned RN for Summit Manor Rehab & Wellness Center in Kentucky. Join a family-based organization delivering resident-centered care across services from rehab to long-term care.

You’ll lead a team, perform direct nursing duties, and help drive quality outcomes in a supportive environment. The role requires an active RN license, CNA supervision, and strong documentation skills, with LTC experience preferred.

Qualifications

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Familiarity with electronic health records (EHR) systems.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living.
• Supervise nursing assistants and medication aides during shifts.
• Monitor and report changes in residents' condition to physicians and families.
• Develop, implement, and review individualized care plans with the team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges.
• Participate in quality assurance activities and staff education.
