Job summary

LiveWell Homecare Agency, LLC seeks a Staff Registered Nurse (RN) to deliver high-quality patient care in home health settings. You will perform initial assessments, develop and adjust care plans, and document findings accurately according to nursing standards.

Responsibilities include initiating IVs per policy, monitoring IV sites every 4 hours, and communicating with the team while recognizing patient status changes. An active RN license and BLS certification are required.

Qualifications

• Current Basic Life Support (BLS) certification required.
• Active RN license to practice in Illinois.
• Ability to perform clinical assessments and interventions.
• Strong communication, teamwork, safety awareness, and documentation skills.

Responsibilities

• Perform initial patient assessments and develop care plans.
• Modify care plans based on assessments and ensure accurate documentation.
• Monitor IV sites every 4 hours; initiate IVs per policy.
• Ensure documentation standards are met and participate in staff meetings.
• Communicate effectively with colleagues and recognize patient status changes.
• Identify personal learning needs and pursue ongoing education.
