Job summary

Ascension at Home together with Compassus seeks a Clinical Care Partner to coordinate safe, efficient transitions of care for hospitalized patients. The role emphasizes bedside engagement, discharge planning, and collaboration with physicians, case management, and post-acute providers across market hospitals.

This in-person position focuses on improving outcomes, reducing length of stay, and ensuring the right care in the right setting at the right time.

Qualifications

• Education: Associate’s degree in Nursing, Health Sciences, or related field; equivalent healthcare experience accepted.
• Experience: 2–3 years preferred in care coordination, discharge planning, or post-acute care.
• Licenses: Active RN, LMSW, LCSW, or LICSW licensure may be required depending on service line.

Responsibilities

• Coordinate timely, safe hospital discharges and post‑acute care placements.
• Collaborate with physicians, case management, and nursing teams across care settings.
• Educate patients and families on post‑acute options and care pathways.
• Engage bedside teams and participate in rounds to align on care transitions.
