Job summary

Samaritan Health Services’ Good Samaritan Regional Medical Center in Oregon seeks a skilled Social Worker to provide social work and discharge planning services, including psychosocial assessments, brief counseling, and resource coordination.

The role requires RN experience and collaboration with physicians, nurses, and the care team to arrange efficient discharge plans and ensure patient-centered care across diverse settings. BLS is required on hire.

Qualifications

• Three (3) years RN experience required.
• Three (3) years hospital nursing experience preferred.
• Utilization Review or other inpatient Case Management certification preferred.
• Experience or training in insurance benefits or covered services including Medicare and Medicaid preferred.
• Healthcare Provider Level BLS required upon hire.

Responsibilities

• Provides social work and discharge planning services including psychosocial assessments, brief patient/family counseling, support, resources, education and intervention.
• Arranges and communicates efficient, timely and cost-effective discharge plans.
