Job summary

AMN Healthcare is seeking an experienced RN Discharge Planner to coordinate discharge planning for inpatient units. The role focuses on case management, chart reviews, and collaboration with facilities to ensure timely returns or safe home transitions.

The position is located just north of Window Rock, Fort Defiance, AZ, with a 13-week assignment and a standard 8:00 am–5:00 pm schedule. Qualified candidates will hold an unrestricted RN license, BLS, and 2+ years in discharge planning or

Qualifications

• Valid RN license in any U.S. state or territory.
• AHA Basic Life Support certification must be current.
• 2+ years of discharge planning experience.
• RN with clinical Med-Surg/acute care experience.

Responsibilities

• Coordinate discharge planning for age-appropriate in-patients with chronic diseases.
• Maintain chart reviews and tracking for referred patients for admission or discharge.
• Collaborate with other facilities to ensure safe transitions home or return to facility.

POSITION SUMMARY – RN Discharge Planner

POSITION DUTIES

– Performs case management and coordinates the discharge planning care process of age-appropriate in-patients with chronic diseases and special health care needs within Multi-Service Unit, Intensive Care Unit, OB Units and Adolescent Care Unit by maintaining a tracking system for referred patients for admission with other healthcare facilities by performing chart reviews for requested services for return to facility or discharge home.

Minimum Required Qualifications –

• Valid and unrestricted Registered Nurse license in any U.S. State or Territory.
• Valid American Heart Association (AHA) certification in Basic Life Support (BLS) and must maintain certification.
• 2+ years experience in discharge planning and Med-Surg experience.
• RN experience in a clinical, medical/surgical, acute and/or critical care setting.

LENGTH OF ASSIGNMENT

13 weeks

SHIFT / HOURS PER WEEK

Monday to Friday, 8:00 a.m. to 5:0 p.m. with one-hour lunch (8-Hour Shift)
